In this example, the FMEA table lists the different process steps involved in a hypothetical food manufacturing process and the potential failure modes that could occur at each step. It also includes the potential effects of those failures, the severity of those effects, the potential causes of the failures, the occurrence rate of the failures, the current controls in place to prevent the failures, the detection rate of the failures, and the overall RPN (Risk Priority Number) for each failure mode. The RPN is calculated by multiplying the severity, occurrence, and detection ratings together.
